Organ supplements, and the math nobody shows you.

Six liver capsules is about a tenth of a serving of liver. Once you know that number you can check any label on the shelf yourself. Here is what actually survives the arithmetic, what does not, and why the evidence behind the whole category is thinner than the packaging suggests.

Quick answer

Do beef organ supplements work?

They can push up some nutrients you already get easily. They do not really move the needle on the ones that are actually hard to get. At a normal dose they give you a meaningful amount of exactly three things: vitamin B12, preformed vitamin A, and copper. The iron, protein, choline, and CoQ10 claims do not survive the arithmetic. There are no published human trials on organ capsules, and the study the marketing keeps citing is from 1951 and used rats.

I asked someone I work with why they had started taking organ capsules. They said they saw a video and it seemed like it would work.

They have since stopped. They told me they cannot tell the difference either way.

That is roughly the whole conversation I am having about these lately. Searches for organ supplements are up almost 300 percent this year, and beef liver capsules on their own are up 94 percent, so it comes up nearly every week now.

The pitch is good. I understand why it works.

Our great-grandparents ate the whole animal. Liver really is one of the most nutrient dense foods on the planet. Modern eating threw it out. Here is a capsule that gives it back.

I do not have a problem with any of that. Liver is a great food.

What I tell people who ask me is that it can push up some nutrients you already get easily, and it does not really move the needle on the ones that are actually hard to get. Iron being the obvious one.

That was an instinct before it was a conclusion, and I wanted to know whether the instinct held up.

It did. The reason is arithmetic, and nobody selling these will show you the arithmetic. So I ran it.

What a capsule actually holds

Liver is about 70 percent water. Freeze drying pulls the water out and leaves everything else behind, so one gram of dried liver powder is roughly 3.3 grams of fresh liver.

That conversion is the whole ballgame. Once you have it you never need to trust a label again.

A 500 milligram capsule is about 1.65 grams of fresh liver. Roughly half a teaspoon.

Six capsules, which is the top of what most labels suggest, comes to about 10 grams of fresh liver. A standard serving of liver is 85 grams.

So a full day at the high end of the dose is somewhere near a tenth of a serving of liver.

Here is what that delivers, running USDA values for raw beef liver through the same conversion. This is 3,000 milligrams a day of pure freeze-dried liver, which is six 500 mg capsules.

Two caveats before anyone screenshots that table.

Most people do not take six. Labels commonly suggest one or two, which puts you at 500 to 1,000 milligrams. Divide everything above by three or six.

And multi-organ blends are worse on this front, not better. If a capsule is liver, heart, kidney, spleen, and pancreas, the liver fraction might be a fifth of what is in there. The organ carrying the nutritional load gets diluted by four organs that are mostly along for the story.

Three numbers worth something

B12 is the strongest. Six capsules is roughly two and a half times a day's requirement, in a form that does not need to be synthesized in a lab. For anyone with a methylation picture where B12 matters, that is not nothing.

Preformed vitamin A is the one I find most interesting, and it is the one the marketing undersells.

Retinol from liver is already in the form your body uses. Beta carotene from plants has to be converted, and that conversion runs through the BCO1 enzyme. Common variants in BCO1 reduce how efficiently a person makes that conversion, which means two people eating identical amounts of carrots and sweet potatoes can end up in very different places. For a poor converter, a food source of preformed retinol is genuinely useful.

That is a real argument for liver. It is just not the argument anyone is making on the label.

Copper lands at about one RDA. Fine on its own. Hold that thought, because it comes back later as the actual safety issue.

The iron claim does not hold up

Iron is on half these labels and it is the claim that falls apart fastest.

Six capsules give you around half a milligram of iron. The RDA is 18 milligrams for premenopausal women and 8 for men.

One egg has more iron than six liver capsules.

Heme iron from liver is absorbed better than the iron in a plant food or a cheap supplement, and that is true. It does not rescue the number. Better absorption of almost nothing is still almost nothing.

If someone is tired and taking liver capsules for iron, they are not treating anything. They are just not finding out what is actually going on.

The CoQ10 number is worth seeing

Beef heart is one of the best food sources of CoQ10 there is, around 11 milligrams per 100 grams.

Same conversion. A 500 milligram dried heart capsule holds roughly 0.19 milligrams of CoQ10.

Studies on CoQ10 use 100 to 300 milligrams a day.

You would need more than 500 capsules to reach the bottom of a research dose.

I include this because it is the cleanest example of the pattern. The claim is technically true. Beef heart is rich in CoQ10. The dose is off by a factor of several hundred, and the gap between those two facts is where the whole category lives.

Everything rests on a 1951 rat study

This is the part that surprised me most. I went looking for human trials on organ supplements and there essentially are not any.

What there is, cited constantly, is an experiment Benjamin Ershoff published in 1951. Sixty rats, three groups of twenty. Standard diet, standard diet plus B vitamins, or standard diet plus 10 percent desiccated liver. He then had them swim in cold water and timed how long they lasted.

The liver group swam four to nine times longer. Some went past 80 minutes. The controls averaged around 13.

That result became the "anti-fatigue factor," and it has been selling liver tablets for seventy-five years.

James Heathers went back and looked at what the control animals were actually eating. The control diets appear to have had no real iron source, and the protein was casein, which interferes with iron absorption. The controls were very likely anemic. Anemic animals also regulate temperature badly, and the test was cold water. They weighed less than the liver group too.

So the finding can be entirely real and still mean something completely different. Liver did not add a mysterious factor. Liver corrected a deficiency the study design had created.

It has not been replicated in people. Not once, in seventy-five years, on a product category now measured in hundreds of millions of dollars.

"Like supports like" is not a mechanism

The second pillar is the idea that eating an organ supports that same organ in you. Spleen for immunity. Thymus for immunity. Adrenal for energy. Pancreas for blood sugar.

There is no mechanism there. Peptides and hormones in animal tissue get broken down into amino acids in your digestive tract. Your body does not read the return address on them.

AdventHealth names the adrenal fatigue and organ-targeting claims specifically as unsupported. I agree with them, and I do not often find myself agreeing with a hospital blog over the functional world.

The version that actually concerns me is thyroid and adrenal glandulars. Those can contain residual active hormone, and the amount is not standardized between batches. If someone is on levothyroxine and adds a thyroid glandular, there is now a variable in the system with no gauge on it. Especially with Hashimoto's in the picture, I would not do it.

The real risk is copper, and nobody mentions it

Three ounces of actual beef liver contains about 12,400 micrograms of copper.

The RDA is 900. The tolerable upper intake level is 10,000.

A single serving of real liver clears the adult ceiling on its own. That is not a supplement problem, that is a liver problem, and it is why "eat liver weekly" is better advice than "eat liver daily."

The capsules are more moderate. At 3,000 milligrams you are near 1,000 micrograms, roughly one RDA. Reasonable.

The issue is stacking. Liver capsules, plus a multivitamin, plus a mineral complex, plus real liver on Sunday, and now nobody is tracking the total. Copper is not a nutrient most people think to add up, and chronic excess is associated with liver damage and gastrointestinal symptoms.

Preformed vitamin A has the same shape. The upper limit is 3,000 micrograms a day. Capsules alone will not get you there. Capsules plus cod liver oil plus a multi might.

Pregnancy is where I would draw a hard line. Preformed vitamin A above the upper limit during pregnancy is associated with congenital malformations, including of the eye, skull, lungs, and heart. These products are marketed aggressively to women who are pregnant or trying, wrapped in ancestral-nutrition language. If that is you, this is a conversation to have with your OB before you open the bottle.

You cannot verify what is in the bottle

Then there is the question of whether the capsule contains what the label says it does.

I could not find a published assay of liver capsules specifically. What I did find is the nearest neighbor, and it is not reassuring.

DAO supplements are made from porcine kidney. Same category, same manufacturing world. A 2023 analysis measured actual enzyme activity in ten over-the-counter DAO products. Two hit their label claim. Eight came in under it. Two had no detectable activity at all.

That is not proof that liver capsules are mislabeled. It is a signal that "freeze dried organ, trust us" is not a corner of the industry with tight quality control. Third-party heavy metal testing runs several hundred to a thousand dollars per batch, which is exactly why the cheap brands skip it.

If a company will not publish its certificates of analysis, work from the assumption that there aren't any.

What I would want to know first

If someone wants a modest, food-based source of B12, retinol, and copper, and they would rather get it from liver than from a synthetic multivitamin, that is a completely legitimate preference. I am not going to talk anyone out of it.

Buy the pure liver product rather than the five-organ blend. The blend dilutes the only organ doing real work.

Four things I would want on the table before starting:

What gets to me about this category is that the ancestral story is doing all the work while the nutrition label sits right there, checkable, in plain sight.

You do not need a clinical trial to know that a 500 milligram capsule is a gram and a half of liver. You need a calculator.

If you are taking these and you feel better, I am not going to argue you out of it. But I would want to know what actually changed, because whatever it is, it is almost certainly not the iron.

The person who got me looking at this stopped taking them and could not tell the difference. That proves nothing on its own. It is just the most common ending I hear.

Not sure what is actually doing anything in your supplement drawer?

Most people I work with are taking six to ten things and cannot say what any of them are for. The intake goes through the whole drawer against your genetics and what you actually eat. Usually a few things come out, one or two get a dose correction, and the list gets shorter and cheaper.
